FINAL FORMS Instructions(click link)
-Everything must be complete in Final Forms before a student-athlete can participate in tryouts
-You can upload into Final Forms starting on 5/1/24
-Physicals must be dated after 4/15/24 and the boxes regarding being cleared must be checked and signed.
-ALL pages of the physical form must be uploaded, not just the first page.
- Physical Examination Form (Filled out by Physician and must be uploaded into Final Forms)
